What is worth discussing is the fact that millions of women are prescribed hormonal birth control with very little information about what it actually does inside the body. The bleeding you experience during the placebo pill week is not a menstrual period. It is a withdrawal bleed — a response to the sudden drop in synthetic hormones when you stop taking active pills. It tells you nothing about your hormonal health, your cycle regularity, or whether you're ovulating. It is, essentially, a managed bleed that was built into early oral contraceptives to make the pill feel more "natural" and palatable to women and physicians at the time.

It Depletes Key Nutrients Your Body Needs
One of the most underappreciated side effects of hormonal birth control — particularly the pill — is its impact on your nutritional status. The synthetic hormones in birth control affect how your gut absorbs certain nutrients, how your liver metabolizes them, and how quickly they're excreted. The result is a well-documented pattern of nutrient depletion that can contribute to a wide range of symptoms: fatigue, mood changes, hair loss, skin issues, poor immune function, and more.

This doesn't mean everyone on the pill will experience all of these deficiencies. Individual variation is significant, and diet plays a major role in how much of an impact depletion has. But if you are experiencing unexplained fatigue, persistent low mood, frequent illness, hair thinning, or other symptoms while on hormonal birth control, nutrient status is absolutely worth investigating.
The birth control pill can also increase thyroxine-binding globulin (TBG), a protein that binds to free thyroid hormones in the blood. Higher TBG means less free, active thyroid hormone — which can produce hypothyroid-like symptoms (weight gain, fatigue, brain fog, hair loss) even when standard thyroid labs look "normal." The zinc and selenium depletion compounds this by impairing thyroid hormone conversion.
The Birth Control Nutrient Support Protocol
- B-complex: Look for a methylated B-complex (containing methylfolate and methylcobalamin) for better absorption. This is especially important if you have an MTHFR gene variant. Take daily with food.

Your gut microbiome — the community of trillions of microorganisms living in your digestive tract — does far more than manage digestion. When hormonal birth control disrupts that community, the downstream effects are wide-ranging.
Research suggests that hormonal contraceptives, particularly the combined oral pill, can affect the gut in several important ways:
Changes in microbial diversity
Studies show that oral contraceptive use is associated with alterations in gut bacterial composition and a reduction in overall microbial diversity. Specifically, there is evidence of decreased abundance of Lactobacillus species — the beneficial bacteria that support a healthy vaginal microbiome, produce lactic acid to keep pH balanced, and help regulate the estrobolome (the group of gut bacteria responsible for estrogen metabolism). It Can Mask Underlying Conditions
This is one of the most important and underappreciated aspects of hormonal birth control: it is prescribed incredibly frequently as a treatment for symptoms — painful periods, irregular cycles, acne, heavy bleeding — without investigation into what is actually causing those symptoms.
Birth control suppresses the hormonal environment in which many conditions develop and progress. That means symptoms can disappear. But disappearing symptoms do not mean the underlying condition has resolved. When birth control is eventually stopped — whether to conceive, due to side effects, or simply because a woman decides she wants to come off it — the original condition often reasserts itself, sometimes more severely than before.
The conditions most commonly masked by hormonal birth control include:
- PCOS (Polycystic Ovary Syndrome): The pill is the standard first-line treatment for PCOS in adolescents and adults. It regulates the withdrawal bleed, reduces androgen levels (improving acne and hair growth), and gives the appearance of a normalized cycle. But PCOS is fundamentally a condition driven by hormonal imbalances, often rooted in insulin resistance, inflammation, and HPA axis dysregulation. None of these root causes are addressed by suppressing ovulation.
- Endometriosis: Hormonal birth control — particularly continuous-use pills — can reduce endometriosis-related pain and bleeding. But endometriosis lesions continue to exist. Research suggests the disease may continue to progress in some women even while on hormonal suppression, and many women experience a significant worsening of symptoms in the months after coming off birth control.
- Fibroids: Hormonal fluctuations can influence fibroid growth and the bleeding they cause. Hormonal birth control can reduce heavy bleeding associated with fibroids, but it does not shrink them or treat their underlying causes.
- Thyroid disorders: As discussed above, the pill elevates TBG and can suppress free thyroid hormone levels, which may mask early hypothyroidism or make thyroid testing less accurate. Women with undiagnosed thyroid disorders may have symptoms attributed to other causes while on the pill — only for the thyroid issue to become apparent once they stop.
None of this means birth control shouldn't be used. For many people, managing symptoms while working on root cause treatment simultaneously is a valid approach. But that only works if both halves of that equation are actually in play — which requires knowing what's being masked in the first place.
Coming Off It Requires Support
One of the most common things I hear from women is that they came off hormonal birth control expecting a return to normal within a cycle or two — and instead found themselves dealing with months of irregular cycles, acne, mood swings, or no period at all. This isn't a sign that something is permanently broken. It's a sign that your body needs time and support to re-establish its own hormonal rhythm after months or years of suppression.
There are several specific post-pill patterns worth knowing about:
Post-pill amenorrhea
Some women find their period does not return for three months or more after stopping hormonal birth control. This is called post-pill amenorrhea, and it occurs because the HPO axis — the communication pathway between the brain and ovaries that was suppressed by birth control — takes time to resume normal function.
